Support Coordination is a NDIS-funded service that helps Participants understand and use their NDIS Plans effectively. We assist you in finding and connecting with the right providers, coordinating services, managing your budget, and achieving your goals.

Support Coordination is available to NDIS participants whose plans include funding for this service. If you’re unsure whether your plan covers Support Coordination, we can help you check and guide you on what to do next.

There are 3 levels of Support Coordination that can be included in a Participant's plan:

Level 1: Support Connection

Support to help a Participant understand their plan, connect with NDIS providers and community, mainstream and other supports to get the most out of their NDIS Plan. 

Level 2: Coordination of Supports

Support to put in place a mix of supports to increase a Participant's capacity to maintain relationships, manage tasks, live more independently and be included in their community. It builds the Participant's confidence and skills to direct their lives, not just their services. 

Level 3: Specialist Support Coordination

A higher level of support for Participants whose situations are more complex and who need specialist support. A Specialist Support Coordinator will assist Participants to manage challenges in their support environment and ensure a consistent delivery of service. 

Social Connection Supports offers all levels of Support Coordination depending on your needs and how your plan is stated.

Both Plan Management and Support Coordination have to be funded in a Participant's NDIS Plan.

A Plan Manager pays providers for supports delivered. They also monitor the Participant's funds and provide financial report.

A Support Coordinator supports the Participant to understand and implement supports included in their plan. They also support the Participant to build skills and direction.

No additional fees apply beyond what is funded in your NDIS plan. Our charges are in line with the NDIS Price Guide and billed directly to your plan.
